Referral Procedure for Intensive In-Community and Behavioral Assistance Services
The referent and/or family member will call New Jersey Child Behavioral Health Services to request Services. (1-877-652-7624)
Child Behavioral Health Services will require the following demographic, billing and background information:
Child & Guardians’ Names
Social Security Number
Date of Birth of Child
Address, Phone Number(s)
Medicaid Number and Plan Type
Brief verbal description of Need
Referent and/or family member will request that Families Matter completes a Needs Assessment and an Initial clinical evaluation with recommendations for the family.
A care coordinator will contact Families Matter to advise that a referral has been made through Child Behavioral Health Services.
A Clinical Interview and Needs Assessment will be conducted by a licensed/masters degreed Behavioral Health Provider from Families Matter. The Needs Assessment and recommendations will be returned to Child Behavioral Health Services for care authorization.
Once Authorization is received, Intensive In-Community Service will be initiated.
Note: If further services are required, Families Matter will make a request in writing to
Child Behavioral Health Services for additional sessions.
